Submit a Blood Sample Today
If you have whole blood lavender-top EDTA or Yellow-top ACD-A tubes on hand. You can mail in a sample with the downloaded Practitioner Requisition Form today. For self pay patients, please call 800-523-3080 for pricing.


Prescription Form for Patient Kit Requests

Simply print this form, complete the required prescription information, hand it to the patient, and tell them to follow the instructions at the top of the page. When the patient contacts us, we will send the kit directly. Alternatively, you can fax the form to 206-219-4000 with the patient's insurance information, mailing address, and phone number, and we will handle the rest.

For New York residents only, a completed informed consent is also required. Please note that only CYP2C19, CYP2C9 (VKOR) , and CYP2D6 are available in New York.

Specimen Types

  • Blood: 5-10 cc whole blood (lavender-top) EDTA or (yellow-top) ACD-A tubes
  • Buccal: Genelex supplied cheek swab kits, blood is preferred as buccals have a small failure rate.
  • Turnaround Time: Ten days, (STAT turnaround time: five days)
